Job summary

UnitedHealth Group is seeking a Hospice Registered Nurse to provide compassionate, patient-centered care in Johnstown, PA. You will assess needs, coordinate care plans, manage symptoms, and educate families while collaborating with the clinical team.

The role requires an active RN license in the assigned state, CPR certification, and may involve on-call responsibilities. A valid driver’s license and reliable transportation are required.

Qualifications

Responsibilities

• Perform initial nursing assessment on new admissions
• Complete, coordinate, and revise the total plan of care
• Regularly communicate patient progress to the clinical manager, physician, and care team
• Develop, prepare, and maintain individualized patient care progress records
• Record pain/symptom management changes/outcomes as appropriate
• Educate families on patient condition, needs, and care
• Make referrals to other hospice care disciplines as needed
• Perform on-call responsibilities and services to patients/families as assigned
• Perform other duties as assigned

About This Role
