Job summary

UnitedHealth Group through Elite Hospice is hiring a Per Diem RN Case Manager to deliver the Plan of Care for hospice patients in home-based settings. You will perform initial evaluations, coordinate care, document progress, and collaborate with the IDG daily to assure quality patient care.

You'll work in a flexible per diem role with competitive hourly pay ($33.50–$50.20) and comprehensive benefits where eligibility applies, while supporting patients and families in their homes and communities.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current driver's license with access to reliable vehicle or public transportation.
• CPR certification preferred or can be completed within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral.
• Identifies patient/family physical, psychosocial, and environmental needs and re‑assesses as needed, no less than every 15 days.
• Initiates and coordinates the plan of care with accurate clinical notes regarding condition.
• Documents problems, goals, interventions, and patient/family response to hospice care.
• Collaborates with patient/family, attending physician and IDG to provide daily care.

As the Per Diem Registered Nurse Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient. Continuously evaluate personal and professional performance and makes necessary changes to increase productivity and quality of care delivered. Adheres to and supports the policies/procedures/goals/objectives of hospice to assure quality patient care.

Primary Responsibilities:

• Makes the initial nursing evaluation in determining eligibility for hospice services during visit within forty-eight (48) hours of referral
• Identifies the patient's/family's physical, psychosocial, and environmental needs and re‑assesses as needed, no less than every fifteen (15) days
• Initiates and coordinates the plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition
• Documents problems, appropriate goals, interventions, and patient/family response to hospice care
• Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily

Required Qualifications:

• Current unrestricted RN licensure in the state of practice
• 1+ years of clinical experience
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation

Preferred Qualification:

• Current CPR Certification or ability to complete within 90 days of hire
