Job summary

Suncrest Hospice, part of LHC Group and within the Optum family, invites a PRN Registered Nurse to provide hospice care in Knoxville, TN. You will manage Plan of Care delivery, evaluate care quality, and collaborate with a multidisciplinary team to support physical, psychosocial, and spiritual well-being.

The role requires an active RN license in the state, a valid driver's license, and 1+ year of clinical experience; CPR certification preferred.

• Ability to work independently with strong judgment.
• Strong written and interpersonal skills.

Responsibilities

• Provide full nursing responsibility for hospice Plan of Care.
• Perform initial evaluation for eligibility within 48 hours of referral.
• Develop and implement treatment plans in collaboration with the care team.
• Document care delivered and patient/family response.
• Coordinate with patient/family, physician and IDG to deliver daily care.
• Instruct patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes and ensure culturally sensitive care.
