Job summary

Bluegrass Care Navigators seeks a Hospice Flex RN to support Hospice RN Case Managers, ensuring patient needs are met and care remains high-quality across home and facility settings.

The role requires an RN license in Kentucky or via compact state, CPR certification, and flexible availability for on-call rotations. This position offers meaningful work supporting patients and families during challenging times.

Qualifications

• Graduate of an accredited school of registered nursing.
• Currently licensed as a Registered Nurse through the Kentucky Board of Nursing or via a participating Compact State.
• Minimum of one year of general nursing experience preferred; new graduates will be considered.
• Demonstrates strong clinical nursing skills and the ability to maintain professionalism during emotionally challenging situations.
• Provides compassionate, patient-centered care while serving diverse populations with respect and cultural sensitivity.
• Maintains current CPR certification.
• Must be a licensed driver with an automobile in good working order.

Responsibilities

• Provide support to Hospice RN Case Managers to ensure patient needs are met and quality care is maintained.
• Assist with new admissions and manage referrals.
• Complete visits for multiple case managers and participate in on-call rotation.
• Deliver empathetic and compassionate care in home and facility settings.

As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.
