Job summary

Bluegrass Care Navigators is seeking a Hospice Flex RN to support Hospice RN Case Managers in delivering high-quality care in home settings, including private homes and facilities. The role focuses on nursing assessments, symptom management, and compassionate patient soutien in Kentucky.

The position requires a Kentucky RN license (or compact state) and CPR certification, with strong communication and autonomous-work capabilities. On-call rotation may be required.

Qualifications

• Graduate of an accredited nursing program.
• Currently licensed as a Registered Nurse by the Kentucky Board of Nursing or a participating Compact State.
• CPR certification is required.

As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.

Monday - Friday 8:00am-5:00pm, with occasional on call

Position Summary:

The HospiceFlexRNis a diverse roleprovidingsupport to Hospice RN Case Managerstoensure patient needsare metandhigh-qualitycareismaintainedinvarioushome care settings, such asprivate residences, skilled nursing facilities, personal care homes,or wherever the patient calls home.TheHospiceFlexRNis dynamic depending on aregional office’s most urgent needs and can involveproviding care tospecific caseloads,completingvisits for multiple case managers,assistingwithnewadmissionsandmanagingreferrals,andparticipatingin on-call rotationsto ensure continuous patient care.A Registered Nurse in this positionmust possess strong assessment skillsand a thorough understanding of disease processes, as well as the ability to manage symptoms proactively.Excellentcommunication skillsare essential for interacting with patients, families, and interdisciplinary team members.The HospiceFlex RNmust be ableto work autonomously,handle various patient care scenarios withprofessionalism, and provide empathetic and compassionate care in emotionallychallenging situations.This position offers ongoingopportunitiesto make a significant and rewarding difference in patients’ lives during a challenging time.

Position Qualifications:

• Graduateof an accredited school of registered nursing.
• Currently licensed as a Registered NursethroughtheKentucky Board of Nursingor through a participating CompactStateBoard of Nursing.
• Minimum of one (1) year of general nursing experience preferred; new graduate nurses will be considered.
• Demonstrates strong clinical nursing skills and the ability tomaintainprofessionalism during emotionally challenging situations.
• Provides compassionate, patient-centered care while serving diverse populations with respect and cultural sensitivity.
• Adheres to accepted professional, ethical, and legal standards of nursing practice.
• Demonstrates understanding of and commitment to hospice philosophy and the principles of death and dying.
• Maintains current CPR certification.
• Must be a licensed driver with an automobile in good working order, that isinsuredin accordance withstateminimumrequirements.
