Job summary

Bluegrass Care Navigators’ Hospice Team member provides care to patients and families choosing comfort-focused treatment at home, nursing homes, or hospice centers. The HospiceFlexRN supports RN Case Managers across diverse home care settings and may handle multiple caseloads or admissions.

The role requires strong assessment skills, empathy, and independent work in emotionally challenging situations, with on-call rotation and a Monday–Friday schedule in Kentucky.

Qualifications

• Graduate of an accredited school of registered nursing.
• Currently licensed as a Registered Nurse through the Kentucky Board of Nursing or through a participating Compact State Board of Nursing.
• Minimum of one (1) year of general nursing experience preferred; new graduate nurses will be considered.
• Demonstrates strong clinical nursing skills and the ability to maintain professionalism during emotionally challenging situations.
• Provides compassionate, patient-centered care while serving diverse populations with respect and cultural sensitivity.
• Adheres to accepted professional, ethical, and legal standards of nursing practice.
• Demonstrates understanding of and commitment to hospice philosophy and the principles of death and dying.
• Maintains current CPR certification.
• Must be a licensed driver with an automobile in good working order, that is insured in accordance wit state minimum requirements.

Responsibilities

• The HospiceFlexRN is a diverse role providing support to Hospice RN Case Managers to ensure patient needs are met and high-quality care is maintained in various home care settings, such as private residences, skilled nursing facilities, personal care homes, or wherever the patient calls home.
• The HospiceFlex RN is dynamic depending on a regional office’s most urgent needs and can involve providing care to specific caseloads and completing visits for multiple case managers.
• Assisting with new admissions and managing referrals, and participating in on-call rotation to ensure continuous patient care.
• The Registered Nurse in this position must possess strong assessment skills and a thorough understanding of disease processes.
• The RN should communicate effectively with patients, families, and interdisciplinary team members and work autonomously.

As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.
