Lakeview Hospital's Hospice team is hiring a Registered Nurse to join our caring program. You will perform skilled nursing assessments in the Homecare setting, develop care plans with clients and families, and coordinate with the interdisciplinary team to meet goals.
Supervision of LPNs and HHAs is included. This on-call, day-shift role requires MN or WI RN licensure, an Associate's degree, and the ability to work independently in patients' homes.

Provides skilled nursing assessments of client/caregivers in the Homecare setting which includes physical, psychosocial and environmental assessment and care. Develops a care plan in consultation with the client /family unit, the interdisciplinary team, and the primary physician. Coordinates and plans with the professionals and caregivers involved, to meet the goals of the client’s plan of care. Supervision of LPNs and HHAs.
On Call - Day shift, 8a-4:30p as needed. Minimum of 32hr/month availability.
