Job summary

Lakeview Hospital is seeking a Registered Nurse for Homecare on a casual, part-time basis. The role focuses on delivering direct and indirect patient care in the home, developing care plans with physicians and families, and coordinating a multidisciplinary team to meet the care goals.

Requirements include MN or WI RN license at start, current BLS certification, and a valid driver's license with reliable vehicle. Casually scheduled evenings or weekends may apply.

Qualifications

• Current BLS – American Heart Association or American Red Cross (CPR/AED for Professional Rescuer & Health Care Provider)
• Must have a dependable vehicle, valid driver’s license and meet Lakeview’s automobile requirements

Responsibilities

• Coordinate all healthcare team members including client/caregiver to achieve the goals of the Care Plan.

Work Schedule: 0 hrs. weekly/0.0FTE Casual; Mon-Fri 8-4:30 pm, Weekend Rotation Minimum 40 hour per month availability required

Required Qualifications

• Associate’s degree in nursing
• Must have MN or WI Registered Nurse (RN) license upon start date.
• • Must provide proof of MN and WI Registered Nurse (RN) license within 4 months of start, if both MN and WI Registered Nurse (RN) licenses are not present on start date.
• Current BLS – American Heart Association or BLS – American Red Cross (CPR/AED for Professional Rescuer & Health Care Provider)
• Must have a dependable vehicle, valid driver’s license, auto insurance and meet all criteria required under Lakeview’s Automobile Liability Guidelines

Preferred Qualifications

• Bachelor’s degree in Nursing
• Proof of application for alternate state within 4 weeks of start, if both MN and WI Registered Nurse (RN) licenses are not present on start date.
