Job summary

First Choice Hospice is seeking a Registered Nurse (RN) to provide compassionate, patient-centered care for community-based hospice patients in the Boise, ID area and surrounding communities.

As part of Jet Health, Inc., you will follow individualized care plans, perform medication administration, wound care, and assessments, and coordinate with the clinical team to ensure comfort, independence and dignity for geriatric patients.

Qualifications

• Current State or Compact RN license.
• Minimum of one-year clinical experience preferred.
• Knowledge of current nursing practices and safety.
• Valid driver's license and auto liability insurance.
• HCHB EMR knowledge is desirable.

Responsibilities

• Follow a designated care plan tailored to patient needs.
• Make home visits to clients in Boise and surrounding areas.
• Administer medications, wound care, treatments and procedures.
• Monitor client conditions and report changes to supervisor.
• Document doctor orders, client assessments and interventions.
• Coordinate case management and care with the clinical team.

Description: First Choice Hospice is looking for Registered Nurses (RN) who is interested in caring for our community-based patients enrolled in our hospice program in the Boise, ID and surrounding areas.

Registered Nurse (RN) Responsibilities

• Follow a designated care plan in accordance with patient's needs
• Make home visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients' conditions, reporting changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors' orders
• Perform client assessments
• Case management and coordination
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications

• Current State or Compact Registered Nurse (RN) License in hand
• A minimum of one-year clinical experience is preferred
• Demonstrate knowledge and skillset in current nursing practice
• Possess a valid state driver's license and automobile liability insurance
• HCHB EMR knowledge is desired
