Job summary

VitalCaring is seeking a dedicated Hospice Registered Nurse (RN) to lead clinical care in patients' homes and facilities. You will guide symptom management, coordinate plans of care, and provide compassionate support to patients and families.

Qualifications

  • Active RN license (or compact eligibility).
  • Graduate of an accredited nursing program.
  • Minimum 1 year of clinical nursing experience.
  • Strong clinical judgment and compassionate care approach.
  • Ability to manage a caseload independently.
  • Excellent communication skills in sensitive situations.
  • Basic EMR proficiency.
  • Valid driver’s license and reliable transportation.

Responsibilities

  • Lead patient-centered hospice care by developing and managing individualized, comfort-focused care plans.
  • Deliver skilled nursing care with a focus on pain and symptom management.
  • Act as case manager, coordinating care across the interdisciplinary team.
  • Collaborate with physicians, social workers, chaplains, and aides.
  • Monitor patient condition and adjust care plans to maintain comfort.
  • Provide education, emotional support, and guidance to patients and families.

Role Overview

As a Hospice RN at VitalCaring, you are more than a nurse - you are the clinical leader, advocate, and steady guide for your patients and their families.

You’ll lead the plan of care, manage symptoms, and ensure every patient receives personalized, comfort-focused care aligned with their goals and wishes. Through each visit, you bring not only clinical expertise, but also reassurance, clarity, and compassion.

What You’ll Do
Lead Patient‑Centered Hospice Care
  • Conduct comprehensive patient and family assessments
  • Develop and manage individualized, comfort-focused care plans
  • Deliver skilled nursing care with a focus on pain and symptom management
Serve as the Care Coordinator
  • Act as case manager, coordinating care across the interdisciplinary team
  • Collaborate with physicians, social workers, chaplains, and aides
  • Monitor patient condition and adjust care plans to maintain comfort
Support Patients & Families
  • Provide education, emotional support, and guidance throughout the journey
  • Help families navigate complex decisions with empathy and clarity
  • Build meaningful, trusting relationships in deeply personal moments
Drive Quality & Compliance
  • Maintain accurate, timely documentation
  • Participate in IDT meetings and care planning
  • Support quality and compliance standards
What You Bring
  • Active RN license (or compact eligibility)
  • Graduate of an accredited nursing program
