The Judge Group is seeking an experienced Registered Nurse (RN) to join our Home Health team in Lorain, OH. The role focuses on providing skilled nursing care in patients' homes, with independent work and meaningful impact.

Responsibilities include assessments, care planning, OASIS familiarity, and coordination with physicians and the care team. Competitive pay, mileage reimbursement, and benefits are offered; schedule is full-time, Monday–Friday.

Formación

Active RN license required.
ADN or higher.
Experience in Home Health nursing.
OASIS proficiency preferred.
Excellent communication, organization, and documentation skills.

Responsabilidades

Perform skilled nursing visits in patients' homes.
Complete OASIS Start of Care, Resumption of Care, Recertification, Transfer, and Discharge assessments.
Assess patient conditions and develop, implement, and update individualized care plans.
Coordinate care with physicians, therapists, caregivers, and interdisciplinary team.
Educate patients and families regarding disease management, medications, treatments, and home safety.
Ensure accurate, timely, and compliant clinical documentation.
Monitor patient progress and communicate changes in condition to the appropriate providers.
Maintain quality standards and regulatory compliance throughout patient care.

Full-Time | Lorain, Cuyahoga & Lake Counties, OH Our client is seeking an experienced and compassionate Registered Nurse (RN) to join our Home Health team. This role is ideal for a clinician who enjoys working independently while making a meaningful impact on patients in their homes.

Pay: $50/hourly (Mileage Reimbursement)

Position Summary

The Home Health RN is responsible for providing skilled nursing care to patients in the home setting, completing comprehensive assessments, and developing individualized plans of care. This position requires strong clinical judgment, excellent documentation skills, and experience completing OASIS assessments.
