Visiting Nurse Association of Ohio Home Health is seeking an experienced RN Case Manager to join the Cleveland Home Health team. You will conduct patient assessments, develop individualized care plans, and coordinate services with an interdisciplinary team, performing in-home visits and maintaining EMR documentation.

The role offers $39.00–$46.00 per hour, a $5,000 sign-on bonus, and a full-time schedule (40 hours/week). Ohio RN license, BLS, and 2+ years in home health are required.

We're looking for an experienced RN Case Manager to join our Home Health team inCleveland, offering top-quartile pay, a $5,000 sign-on bonus, and the tools and support to do your best work every day.

Compensation

• $39.00 – $46.00 per hour / productivity point, depending on experience
• $5,000 sign-on bonus
• Full-time / 40 hours per week

Service Area & Schedule

Cuyahoga County | Monday through Friday, 8am - 4:30pm

What You'll Do

• Conduct comprehensive patient assessments and develop individualized, evidence-based care plans tailored to each patient's needs
• Coordinate services across an interdisciplinary team to ensure all aspects of care are efficiently managed
• Perform home visits, monitor patient progress, and adjust care plans as needed
• Educate patients and families on health management, medications, and available resources
• Serve as the primary point of contact for patients, families, and referring providers
• Maintain accurate and timely documentation in the EMR, including OASIS assessments, within required timeframes
• Ensure compliance with Medicare, Medicaid, and agency regulations
• Participate in team meetings, case conferences, and ongoing professional development

Requirements

Must-Have:

• Active RN license (Ohio) + current BLS certification
• Associate or Bachelor's degree in Nursing (BSN preferred)
• Minimum 2 years of clinical nursing experience, including at least 1 year in a home health setting
• Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland service area
• Familiarity with Medicare and Medicaid regulations
• Proficiency in electronic medical record (EMR) systems and home health-specific software

Preferred / Nice-to-Have:

• OASIS certification or demonstrated OASIS experience
• Familiarity with HCHB or similar home health EMR
• Strong caseload management and time management skills
• Experience with care coordination across interdisciplinary teams
