Job summary

Better at Home is seeking a PRN Home Health RN to travel across Lake, Lorain, Cuyahoga, Medina, and Geauga counties from our Mentor office. You will conduct in-home assessments, develop care plans, and ensure patients meet program criteria, all on a flexible PRN schedule.

You must hold an active Ohio RN license, have reliable transportation, and bring home health or Medicaid experience. This role offers meaningful patient interaction and autonomy in a field setting.

Qualifications

• Experience in home health or Medicaid populations preferred.

Job Summary

Join our dedicated healthcare team as a PRN Home Health Registered Nurse (RN) specializing in Client Assessments!

Based out of our Mentor office, you will be a true \"road warrior,\" traveling to patients' homes across a 5-county territory to evaluate their needs, develop care plans, and ensure they meet the criteria for their programs. This PRN position offers ultimate flexibility-you tell us when you are available, making it the perfect role to supplement your current income while providing critical, one-on-one care to a vulnerable patient population.

Duties

• Client Assessments: Conduct comprehensive in-home initial assessments, routine reassessments, and recertifications for private pay and Medicaid waiver patients.
• Care Planning: Develop, implement, and update individualized plans of care based on the patient's physical, emotional, and environmental needs.
• Territory Navigation: Drive efficiently across Lake, Lorain, Cuyahoga, Medina, and Geauga counties to reach patients in their homes.
• Clinical Documentation: Maintain accurate, timely, and fully compliant electronic medical records (EMR) for all assessments in accordance with state and Medicaid guidelines.
• Care Coordination: Collaborate seamlessly with internal clinical supervisors, physicians, and care team members to ensure continuity of care.

What We Are Looking For:

• Licensure: Active, unencumbered Registered Nurse (RN) license in the State of Ohio. (Please note: Due to assessment regulations, LPNs do not qualify for this specific role).
• Mobility: Valid driver's license, active auto insurance, and a highly reliable personal vehicle are mandatory.
• Territory Comfort: Willingness and ability to drive extensively across the 5-county service area.
• Experience: Previous experience in home health, community nursing, or working with Medicaid populations is strongly preferred.
• Skills: Strong clinical judgment, excellent time management, and the ability to work independently in a field environment without direct onsite supervision.
