Job summary

Acclaim Home Health is seeking an experienced Registered Nurse (RN) in Milford, MA to join our home health team. This role offers flexible scheduling, autonomy, and strong earning potential with the possibility of six figures in the first year.

You will deliver personalized, high-quality nursing care in patients’ homes, perform assessments, educate families, and collaborate with an interdisciplinary team to optimize outcomes.

Qualifications

• Current RN license in the applicable state.
• Home health experience preferred.
• Strong clinical assessment, critical-thinking and decision-making skills.
• Excellent communication and interpersonal skills.
• Ability to work independently while collaborating with a care team.
• Strong organization and time-management skills.
• Valid driver's license and reliable transportation.

Responsibilities

• Conduct comprehensive nursing assessments and develop individualized plans of care.
• Provide skilled nursing services based on each patient's needs and physician orders.
• Monitor patient conditions, identify changes, and communicate concerns.
• Educate patients and caregivers on medications and disease management.
• Collaborate with physicians, therapists, care coordinators, and the team.
• Manage and prioritize an assigned patient caseload with high care standards.
• Complete timely, accurate, and compliant clinical documentation.
• Participate in care coordination to ensure positive patient outcomes.
