Qualifications

• Current Registered Nurse (RN) license in the applicable state.
• Home health experience preferred.
• Strong clinical assessment, critical-thinking, and decision-making skills.
• Excellent communication and interpersonal skills.
• Ability to work independently while collaborating with a care team.
• Strong organization and time-management skills.
• Valid driver's license and reliable transportation.

Responsibilities

• Conduct comprehensive nursing assessments and develop individualized plans of care.
• Provide skilled nursing services based on each patient's needs and physician orders.
• Monitor patient conditions and communicate changes to the team.
• Educate patients and caregivers on medications, disease management, and independence strategies.
• Collaborate with physicians, therapists, care coordinators, and other team members.
• Manage and prioritize an assigned patient caseload with high standards of care.
• Maintain timely, accurate, and compliant clinical documentation.
• Participate in care coordination to ensure positive patient outcomes.

What You'll Do
