Job summary

Acclaim Home Health is seeking a skilled Registered Nurse (RN) to join our growing home health team in Worcester, MA. This role offers greater flexibility, professional growth, autonomy, and strong earning potential, with a $500 sign-on bonus and bi-weekly pay.

You will conduct comprehensive nursing assessments, provide skilled nursing services, and educate patients and caregivers to improve health and independence. Experience in home health and a valid RN license are preferred.

Qualifications

• Current RN license in the applicable state.
• Home health experience preferred.
• Strong clinical assessment, critical-thinking, and decision-making skills.
• Excellent communication and interpersonal skills.

Responsibilities

• Conduct comprehensive nursing assessments and develop individualized plans of care.
• Provide skilled nursing services based on each patient's needs and physician orders.
• Monitor patient conditions, identify changes in health status, and communicate concerns appropriately.
• Educate patients and caregivers on medications, disease management, treatment plans, and strategies for maintaining health and independence.
• Collaborate with physicians, therapists, care coordinators, and other members of the interdisciplinary team.
• Manage and prioritize an assigned patient caseload while maintaining high standards of care.
• Complete timely, accurate, and compliant clinical documentation.
• Participate in care coordination and help ensure positive patient outcomes.
