Acclaim Home Health is seeking a skilled Registered Nurse (RN) to join our home health team in Worcester, MA. You’ll provide personalized, high-quality care in patients’ homes, supporting independence and improved quality of life.

This role offers flexible scheduling, strong earning potential, ongoing professional growth, continuing education, and a collaborative team environment, with bi-weekly pay and a $500 sign-on bonus.

Qualifications

• Current Registered Nurse (RN) license in the applicable state.
• Home health experience preferred.
• Strong clinical assessment, critical-thinking, and decision-making skills.

Responsibilities

• Conduct comprehensive nursing assessments and develop individualized plans of care.
• Provide skilled nursing services based on each patient's needs and physician orders.
• Monitor patient conditions, identify changes in health status, and communicate concerns appropriately.
• Educate patients and caregivers on medications, disease management, treatment plans, and strategies for maintaining health and independence.
• Collaborate with physicians, therapists, care coordinators, and other members of the interdisciplinary team.
• Manage and prioritize an assigned patient caseload while maintaining high standards of care.
• Complete timely, accurate, and compliant clinical documentation.
• Participate in care coordination and help ensure positive patient outcomes.

What We’re Looking For
