Job summary

Acclaim Home Health seeks a skilled Registered Nurse (RN) to join our growing home health team in Framingham, MA. You’ll deliver personalized, high-quality care in patients’ homes, supporting independence and quality of life while benefiting from flexible scheduling and strong earning potential.

Expect to conduct assessments, develop care plans, educate patients and caregivers, and collaborate with physicians and the care team.

Qualifications

• Active RN license in the applicable state (MA preferred).
• Home health experience is preferred.
• Strong clinical assessment, critical-thinking, and decision-making skills.
• Excellent communication and interpersonal abilities.

Responsibilities

• Conduct comprehensive nursing assessments and develop care plans.
• Provide skilled nursing services per physician orders.
• Monitor health status, identify changes, and communicate concerns promptly.
• Educate patients and caregivers on medications and disease management.
• Collaborate with physicians, therapists, care coordinators and the team.
• Manage a patient caseload while maintaining high standards of care.
• Complete timely, accurate clinical documentation.
• Participate in care coordination to ensure positive outcomes.

What You’ll Do

• Conduct comprehensive nursing assessments and develop individualized plans of care.
• Provide skilled nursing services based on each patient’s needs and physician orders.
• Monitor patient conditions, identify changes in health status, and communicate concerns appropriately.
• Educate patients and caregivers on medications, disease management, treatment plans, and strategies for maintaining health and independence.
• Collaborate with physicians, therapists, care coordinators, and other members of the interdisciplinary team.
• Manage and prioritize an assigned patient caseload while maintaining high standards of care.
• Complete timely, accurate, and compliant clinical documentation.
• Participate in care coordination and help ensure positive patient outcomes.

What We’re Looking For

• Current Registered Nurse (RN) license in the applicable state.
• Home health experience preferred.
• Strong clinical assessment, critical-thinking, and decision-making skills.
• Excellent communication and interpersonal skills.
• Ability to work independently while collaborating effectively with a care team.
• Strong organization and time-management skills.
• Valid driver’s license and reliable transportation.
