Pathways Healthcare in Concord, MA is seeking a per diem Registered Nurse to join our physician- and nurse practitioner-led home health and hospice team serving the Lowell area.

You will initiate preventive and rehabilitative nursing procedures, administer medications, assess patients, and collaborate with physicians to create robust Plans of Care. Flexible scheduling supports ongoing professional growth and excellent patient care.

Qualificações

• Must hold an active RN license in the state where the position is based.
• Preferably 2+ years of nursing experience, with a focus on geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsabilidades

• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.
