FMOL Health in Monroe, Louisiana is seeking aRegistered Nurse to provide safe therapeutic care within a holistic, patient- and family-centered framework. The RN will apply knowledge, skills and licensure to deliver care across the continuum, including education for patients and families.

The incumbent will perform assessments, develop plans based on patient trends, coordinate transitional care, and collaborate with the health care team.

Qualifications

• Graduate of accredited school of nursing.
• Current Louisiana State RN license and BLS certification.
• ACLS certification within first 6 months of hire.

Responsibilities

• Clinical practice and care management.
• Perform psychosocial/physiologic assessments.
• Formulate care plan based on patient trends and educate patient/family.
• Plan routine transitional health care needs and discuss options.
• Respond to changes with independent clinical judgment.
• Adapt education to patient/family and collaborate to set outcomes.
• Ensure efficient care delivery and identify priorities.
• Advocate for patient rights and resolve conflicts as needed.
• Document all patient care activities per standards.
• Communicate and collaborate with health care team, patients, and families.
• Delegate care appropriately and support staff assignments for productivity.
• Participate in unit quality improvement initiatives and apply evidence-based practice.
