Responsibilities

• Clinical Practice and Care Management.
• Performs psychosocial/physiologic assessments and integrates changing data.
• Formulates a plan of care and educates patient/family; discharge planning.
• Collaborates with health care team to optimize patient outcomes.
• Maintains documentation per standards and participates in quality initiatives.
• Provides guidance and mentorship to staff; engages in continuous learning.
