Concordia Lutheran Ministries is seeking a Home Health Registered Nurse (RN) to deliver personalized, one‑on‑one patient care in home settings. You will assess and plan care with physicians, patients, and families, and provide treatments such as IV starts and wound care.

You will educate patients and families, monitor conditions, and manage time effectively while maintaining autonomy within a supportive team. A one‑year minimum RN clinical experience is preferred.

• Home health experience is preferred.

As a Home Health RN, you'll be at the heart of this mission.

Conduct thorough assessments and develop individualized care plans in collaboration with physicians, patients, and families.

• Treatments, IV starts, Chest Tube Management, and Wound care.

• Educating and empowering patients and families on managing their health conditions and building confidence in self‑care.
• Utilizing strong clinical assessment skills to monitor patient conditions and identify changes.
