Job summary

Concordia Lutheran Ministries' Concordia Visiting Nurses is seeking a Home Health Registered Nurse (RN) to deliver personalized, one-on-one patient care in home settings.

You will conduct assessments, develop individualized care plans with physicians, patients and families, perform treatments (IV starts, wound care) and educate families to empower self-care, while enjoying flexible scheduling and a supportive team.

Qualifications

• Minimum one year of recent clinical experience as a Registered Nurse recommended.
• Home care experience recommended.
• Adaptability and compassion with a patient-centered focus.
• Ability to work independently and manage time effectively.

Responsibilities

• Conduct thorough assessments and develop individualized care plans in collaboration with physicians, patients, and families.
• Treatments, IV starts, Chest Tube Management, and Wound care.
• Educating and empowering patients and families on managing their health conditions and building confidence in self-care.
• Utilize strong clinical assessment skills to monitor patient conditions and identify changes.
