Job summary

BrightStar Care of Concord, Lexington & Woburn is seeking an experienced RN to lead clinical care for our home health patients. You will oversee care-staff, develop individualized plans, and conduct in-home assessments to ensure high-quality service.

The role requires an active RN license and CPR, with 3–5 years of clinical experience and home health exposure. Competitive base pay, signing bonus, and benefits are offered in a supportive, patient-focused environment.

Responsibilities

• Lead and manage clinical care-staff for assigned home health cases.
• Develop and implement individualized patient care plans.
• Perform in-home assessments and ensure quality care delivery.
• Administer medications, IV infusions, and treatments as prescribed.
• Provide advanced wound care, including NPWT management.
• Maintain accurate documentation and ensure regulatory compliance.
• Supervise and support nurses, aides, and caregivers.
• Coordinate staffing and ensure proper coverage for patient cases.
• Collaborate with physicians and healthcare professionals.

• Total compensation (base salary + Annual bonus): $105,000 – $120,000/year
