Job summary

BrightStar Care of Concord, Lexington & Woburn, MA is seeking an experienced Registered Nurse to lead clinical care for home-based patients. The role focuses on delivering high-quality nursing services in patients' homes and coordinating care with a supportive team.

You will administer medications and IV infusions, develop individualized care plans, and supervise nurses, aides, and caregivers. A strong home health background and CPR certification are required.

Qualifications

• Active RN license (required).
• CPR certification (required).
• 3–5 years of clinical experience; home health agency or similar setting preferred.

Responsibilities

• Lead and supervise clinical care-staff for assigned patient base.
• Develop and implement individualized patient care plans.
• Perform in-home assessments and ensure quality care delivery.
• Administer medications, IV infusions, and treatments as prescribed.
• Provide advanced wound care, including Wound VAC/NPWT management.
• Maintain accurate documentation and ensure regulatory compliance.
• Supervise and support nurses, aides, and caregivers.
• Coordinate staffing and ensure proper coverage for patient cases.
• Collaborate with physicians and other healthcare professionals.

• Total compensation (base salary + Annual bonus): $105,000 - $120,000/year
