Job summary

Ambercare Homecare in New Mexico seeks a Registered Nurse to join our growing team in Belen, NM. This full-time role offers Mon-Fri, 8am–5pm, with patient visits and coordination of care in home-health settings.

You will assess patients, develop plans of care, collaborate with physicians and the interdisciplinary team, and ensure accurate documentation while delivering compassionate, high-quality care to our clients.

Qualifications

• Licensed RN in the state of New Mexico.
• At least 1 year of clinical nursing experience, preferably in home health or related settings.
• CPR certification is required.
• Must have reliable transportation, a valid driver’s license, and appropriate auto insurance.

Responsibilities

• Provide patient visits as assigned, including assessment, planning, implementation and evaluation of the nursing process.
• Gather data on patient and family needs across physical, psychological, social and spiritual factors and coordinate interventions.
• Communicate with attending physician and interdisciplinary team to coordinate optimal care.
• Deliver visits to assess needs and update the plan of care.
• Perform therapeutic, preventive and rehabilitative nursing procedures as directed by the physician.
• Perform clinical pronouncement of death and support families as applicable.
• Maintain accurate patient records, charts, and timely care plans.

RN What You’ll Do:

• Provide visits as assigned by the Clinical Services Manager, including the assessment, determination of eligibility, admissions processing, planning, implementation and evaluation phases of the nursing process. Assignments will vary depending on licensure.
• Obtain data and assess necessary information from patient physical, psychological, social and spiritual factors that may impact patient and family's needs and coordinate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team and other agencies for the purpose of coordinating optimal care
• Provide visits to assess the patient's needs and update the plan of care
• Perform therapeutic, preventative and rehabilitative nursing procedures as directed by the attending physician
• Perform clinical pronouncement of death and provide support to family, as applicable to licensure.
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely

Qualifications:

• Registered Nurse
  • Graduate from an accredited registered nursing program and licensed in the state
  • At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation or medical surgical nursing (preferred)
  • Home health experience preferred
  • Possess and maintain valid CPR certification.
  • Must have reliable transportation, current driver's license and appropriate automobile insurance
