Responsibilities include performing and documenting initial and ongoing assessments, developing care plans with the interdisciplinary team, administering medications, and monitoring patient responses to interventions as part of a compassionate health

• Perform and document accurate initial and ongoing assessments to evaluate patient conditions and needs.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team to address identified patient needs and goals.
