Overview
Monitors and assesses the delivery of home health field services. Supervises the activities of the field Home Health Aides (HHA). Provides case management support, clinical intervention, development of care plans, and follow-up as needed. Works under general supervision.

What You Will Do
- Perform initial and ongoing supervisory field visits to patients’ homes for assessment, training support, in-home coordination, and crisis management.
- Convey significant changes in patient’s condition, emergency interventions, or care plan updates.
- Report interventions to the clinical manager and document findings electronically.
- Collaborate with primary care physicians and the clinical team to perform comprehensive patient assessments, including medical history and medication review.
- Supervise initial home patient assessments to ensure HHA adherence to VNS guidelines and regulations.
- Identify ongoing training needs of HHAs and document electronically.
- Deliver the start of care packet and review it with the patient/caregiver during the initial visit.
- Educate and review the paraprofessional plan of care with the HHA.
- Participate in quality assurance and educational programs as requested by management.
- Participate in special projects and perform other duties as assigned.
Qualifications
Licenses and Certifications:
- Current license and registration to practice as a Registered Professional Nurse in New York State required.
- CPR/BLS Certification required.
Education:
- Associate's Degree in Nursing from an approved program required.
- Bachelor's Degree in Nursing from an approved program preferred.
Work Experience:
- Minimum of two years of clinical experience as a Registered Nurse required.
- Home care experience in a licensed home care service agency or CHHA setting preferred.
Pay Range
USD $40.95 - USD $51.19 per hour.
