RN Field Supervisor/Clinical Educator (Per Diem 1099 Contract or W2 Employee)

MHC Marks Home Care Agency is seeking a compassionate and skilled Registered Nurse Field Supervisor/Clinical Educator to join our team. We offer the flexibility of either a per diem 1099 contract position or a full-time W2 employee option. In either role, you will be responsible for supervising and training home health aides, personal care aides, and nurses, as well as overseeing clinical education programs to ensure high standards of care for our patients.

Key Responsibilities:

• Perform comprehensive assessments of newly referred patients to determine health needs and appropriate care levels.
• Develop individualized care plans based on patient assessments and collaborate with physicians to implement care strategies.
• Supervise and provide on-the-job training to clinical staff, including nurses and paraprofessional staff.
• Conduct regular home visits for patient re-assessments and adjust care plans as necessary.
• Provide continuing education and competency testing for clinical staff to ensure quality care and adherence to agency protocols.
• Document patient care, progress, and outcomes in compliance with agency policies.
• Collaborate with physicians, families, and other healthcare professionals to ensure optimal patient outcomes.
• Assist in discharge planning and ensure a smooth transition of care.
• Maintain detailed and accurate clinical documentation of patient progress.
• Perform competency reviews and training activities for clinical staff.

Minimum Qualifications:

• Active and unrestricted NYS RN license and Registration
• Malpractice Insurance
• Strong written and verbal communication skills
• Ability to work independently and as part of a collaborative team
• Proficiency with Microsoft Office Suite and other computer programs
• Authorization to work in the United States (must provide documentation as required)
