Job summary

Spectrum Healthcare Resources seeks a dedicated Registered Nurse – Disease Manager to support chronic disease management and preventive care at Joint Base Elmendorf-Richardson, AK. You will help patients improve health, manage chronic conditions, and achieve better long-term outcomes.

Position highlights include a full-time schedule, Monday–Friday, and a benefits package with medical, dental, vision, 401(k), PTO and sick leave. U.S. citizenship and BSN are required.

Qualifications

• BSN from ACEN- or CCNE-accredited program.
• Current, unrestricted RN license in the U.S.
• 2 years of full-time disease management and care coordination experience in last 3–4 years.
• Experience managing chronic conditions (diabetes, hypertension, COPD, heart failure).
• Current BLS certification.
• Strong patient education and care coordination skills.

Responsibilities

• Develop and monitor chronic disease management plans.
• Educate patients on disease prevention and self-management.
• Coordinate care with providers and multidisciplinary teams.
• Track patient progress and support improved health outcomes.

Position Requirements

• U.S. Citizenship
• Bachelor of Science in Nursing (BSN) from an ACEN- or CCNE-accredited program
• Current, unrestricted RN license from any U.S. jurisdiction
• 2 years of full-time Disease Management and Care Coordination experience within the last 3–4 years
• Experience managing chronic conditions such as diabetes, hypertension, COPD, or heart failure
