Job summary

Spectrum Healthcare Resources seeks an experienced Inpatient RN Case Manager to support patient care at Joint Base Elmendorf-Richardson, Alaska. This full-time role focuses on care coordination, discharge planning, and successful transitions for military patients.

The position requires U.S. citizenship, an RN license, and CCM/CDMS/CRRN or approved certification, with 3 years of RN Case Management in the last 4 years.

Qualifications

• U.S. Citizenship required.
• ADN or higher from ACEN/CCNE-accredited program.
• Current, unrestricted RN license in the U.S.
• 3 years RN Case Management experience within the last 4 years.
• Case Management certification (CCM, CDMS, CRRN, or PWS-approved).
• Current BLS certification.

Responsibilities

• Coordinate inpatient treatment and discharge planning.
• Assess patient needs and develop individualized care plans.
• Collaborate with physicians, nurses, and social workers.
• Facilitate transitions to outpatient and community services.

Position Requirements
