Spectrum Healthcare Resources, Inc. seeks a Registered Nurse – Disease Manager to support chronic disease management and preventive care at Joint Base Elmendorf-Richardson, AK.

This full-time role offers a Monday–Friday schedule (7:30 AM–4:30 PM) with comprehensive benefits including medical, dental, vision, 401(k), PTO, and sick leave. U.S. citizenship, a BSN from an ACEN/CCNE-accredited program, an active RN license, and BLS are required.

Qualifications

• U.S. Citizenship is required.
• BSN from ACEN- or CCNE-accredited program.
• Current, unrestricted RN license in the U.S.
• 2 years of full-time disease management and care coordination experience in the last 3–4 years.
• Experience managing chronic conditions such as diabetes, hypertension, COPD, or heart failure.
• Current BLS certification.
• Strong patient education and care coordination skills.

Responsibilities

• Develop and monitor chronic disease management plans.
• Educate patients on disease prevention and self-management.
• Coordinate care with providers and multidisciplinary teams.
• Track patient progress and support improved health outcomes.
