Job summary

Human Touch Home Health in Richmond, VA seeks an experienced Registered Nurse Clinical Manager for a full-time, in-office leadership role. You will lead a diverse team of clinicians and support staff, driving quality outcomes and a culture of excellence.

Ideal candidate will mentor clinicians, review clinical documentation, ensure COP compliance, and partner with physicians to advance patient care in the Richmond office.

Qualifications

• Active Virginia RN License required.
• Experience in Home Health clinical management or supervisory role preferred.
• Strong knowledge of OASIS, Medicare Regulations, and Conditions of Participation.
• Experience with WellSky/Kinnser or similar Home Health EMR preferred.

Responsibilities

• Oversee daily clinical operations of the Richmond office.
• Supervise, mentor, and support nurses, therapists, social workers, and home health aides.
• Review OASIS assessments, plans of care, and clinical documentation.
• Ensure compliance with Medicare, Medicaid, COPs, and accreditation standards.
• Monitor quality metrics and lead performance improvement initiatives.
• Collaborate with physicians, referral partners, and interdisciplinary team members.
• Conduct case conferences, clinical audits, and staff coaching.
• Support employee engagement, retention, and professional development.

• Experience with WellSky/Kinnser or Similar Home Health EMR Preferred
• Excellent Leadership, Communication, Organizational, and Problem-Solving Skills
• Passion for Coaching, Developing, and Supporting Clinical Teams
