Job summary

Lancesoft in Florida is seeking a Clinical Manager to oversee direct patient care by clinical staff and ensure high-quality, compliant service delivery. The role involves coordinating assessments, plans of care, and physician communication to meet regulatory standards.

Ideal candidates hold an RN license, CPR, and Medicare home health experience with OASIS. Two years RN practice with management exposure in home care or hospice is preferred, with ongoing hiring and staff development

Qualifications

• Graduate of an accredited School of Nursing.
• Current Registered Nurse (RN) license in the applicable state.
• Current CPR certification.
• Valid driver's license, auto insurance, and reliable transportation.
• Medicare Home Health experience, including hands-on experience with OASIS assessments, documentation, and compliance requirements.
• Minimum of two years of Registered Nurse experience, including at least one year of management experience in home care, hospice, or an equivalent healthcare setting.

Responsibilities

• Coordinates and oversees all direct patient care services provided by clinical personnel, ensuring quality care, regulatory compliance, and appropriate clinical documentation.
• Conducts and delegates patient assessments and reassessments, updates plans of care, and interprets patient needs in accordance with company policies, physician orders, and applicable healthcare regulations.
• Ensures patients meet admission criteria and determines eligibility for admission to services.
• Assigns appropriate clinicians to patient cases as needed.
• Assists clinicians in establishing immediate and long-term therapeutic goals, setting priorities, and developing patient Plans of Care (POCs).
• Coordinates communication among team members, attending physicians, patients, and caregivers to ensure appropriate care delivery and effective outcome planning.
• Supervises assigned clinical employees and oversees their performance, coordination, and overall direction in accordance with company policies and procedures.
• Participates in interviewing, hiring, training, and developing direct care clinicians.
• Evaluates employee performance, provides coaching, recommends in-service education, and ensures compliance with internal policies and clinical standards.
• Reviews clinical documentation, including Start of Care (SOC), Resumption of Care (ROC), and recertification assessments, to ensure appropriateness of care, quality of service delivery, and compliance with documentation requirements.
• Ensures compliance with Medicare Home Health regulations, including OASIS (Outcome and Assessment Information Set) assessment, documentation, and submission requirements.
