Job summary

Qualifications

• Graduate of an accredited School of Nursing.
• Active RN license and CPR certification.
• Minimum of 2 years RN experience; at least 1 year in management in home care, hospice, or similar setting.
• Medicare Home Health experience including hands-on OASIS assessments, documentation, and compliance.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Oversee and coordinate direct patient care services.
• Review patient assessments, reassessments, and Plans of Care (POCs).
• Ensure compliance with Medicare Home Health regulations and OASIS documentation requirements.
• Coordinate care with clinicians, physicians, patients, and caregivers.
• Supervise, mentor, and evaluate clinical staff.
• Participate in recruitment, onboarding, training, and staff development.
• Review Start of Care (SOC), Resumption of Care (ROC), and recertification documentation.
• Promote clinical excellence, regulatory compliance, and positive patient outcomes.
