Role: Registered Nurse, Case Manager (RNCM)

Overview

This is an opportunity for a dedicated Registered Nurse Case Manager to join our client and make a difference in the lives of patients in hospice care. You will be part of a supportive team, providing essential care and coordination for patients and their families.

Schedule and Shift Details

M-F ; 8a-5p (Start and end times vary from day to day – expected to be available from 8am – 5pm daily.) o Weekends – (1) 24 hour weekend day shift per month AND (2) weekday on calls per month (1) 24 hour weekend day per month: 8A to 5P on Saturday or Sunday; will have scheduled visits ahead of time & seeing patients in the field – beginning 5pm until 8am, will be a true on call.

FYI: We do also have dedicated After Hours RN calls off. (always 3 dedicated after hour RNs on every night) Additional stipends and compensation for fulfilling on call (2) weekdays of 5P to 8A (2) holidays per year – 1 summer & 1 winter; 3 floating holidays
