Crossbridge Hospice, LLC is seeking an RN Case Manager to provide end-of-life hospice care to patients in home or community settings. You will supervise hospice aides, coordinate care plans, and communicate with families with compassion and clinical expertise.
The role requires an active RN license, a valid driver's license, and at least two years of RN experience with hospice/palliative nursing preferred. Schedule is Monday through Friday, 8am-5pm, with on-call responsibilities as needed.

The RN Case Manager is responsible for providing end-of-life care and hospice services to patients in their home or community setting. This includes intermittent skilled nursing services and directs, supervises, and instructs hospice aide staff in the provision of personal patient care.
This role requires compassion, strong clinical judgment, excellent communication skills, and the ability to support patients and families through highly sensitive and meaningful experiences.
Schedule: Mon-Fri 8am-5pm. On call as needed.
