Registered Nurse Case Manager - Direct Hire

Location: McHenry, ILSchedule: Monday-Friday, 8:00 AM-5:00 PM (daily start/end times may vary)On-Call Requirements:

• One (1) 24-hour weekend day shift per month
• Two (2) weekday on-call shifts per month (5 PM-8 AM)
• Two (2) holidays per year (1 summer, 1 winter)
• Three (3) floating holidays
• Additional stipends and compensation provided for on-call coverage
• Dedicated after-hours RN team (3 RNs nightly)

This is an excellent opportunity for a compassionate and experienced Registered Nurse Case Manager to join a supportive hospice team in McHenry, IL. In this role, you will provide essential care coordination, leadership, and patient advocacy for individuals receiving hospice services. You'll work closely with interdisciplinary teams to ensure patients and families receive exceptional end-of-life care.

Required Qualifications

Education:

Licensure:

• Valid Registered Nurse (RN) license in the state of Illinois (IL)

Certifications (ALL required):

• American Heart Association Basic Life Support (AHA BLS)
• American Heart Association Advanced Cardiac Life Support (AHA ACLS)

Schedule & Shift Details

• Monday-Friday, 8:00 AM-5:00 PM (flexible daily start/end times)
• On-Call:
  • One 24-hour weekend day per month
  • Two weekday on-call shifts (5 PM-8 AM)
  • Two holidays per year (1 summer, 1 winter)
  • Three floating holidays
• Dedicated after-hours RN team supports overnight coverage

Travel Requirements

• Local travel required across multiple counties
• Must be comfortable working in field-based hospice environments
