Job summary

Bristol Hospice in Salem, OR is looking for a dedicated Registered Nurse Case Manager who will provide compassionate, whole-person care and advocate for patients and families throughout the hospice journey.

This full-time, hourly position offers a daytime schedule, competitive pay of $50.00–$54.00 per hour, and a comprehensive benefits package including health, retirement, and mileage reimbursement. You will join a mission-driven team delivering high-quality end-of-life care.

Qualifications

• Current RN license in good standing
• Valid driver's license and reliable transportation
• CPR certification (current)
• Strong observational and communication skills
• Flexible, team-oriented attitude

Responsibilities

• Develop and implement individualized care plans based on current and prior medical history
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure
• Administer physician-ordered medications and treatments with skill and empathy
• Educate patients and their families on care techniques and symptom management
• Track and communicate changes in patient condition, ensuring timely and effective responses
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery
• And contribute to other supportive duties as part of a compassionate, mission-driven team
• And additional duties as assigned

What You’ll Bring to The Team:

• A valid Registered Nurse license in good standing in the state you will work in
• A valid driver’s license and reliable transportation
• CPR certification (current)
