Job summary

Bristol Hospice in Redding, CA is seeking a dedicated RN Case Manager who will advocate for patients and families, coordinating care across disciplines to ensure comfort and dignity throughout the hospice journey.

You will perform assessments, develop individualized care plans, monitor vital signs, administer medications, educate families, and document progress with empathy.

Qualifications

• Current RN license in good standing (CA).
• CPR certification (current).
• Valid driver's license and reliable transportation.
• Excellent written and verbal communication.
• Ability to work with interdisciplinary teams.

Responsibilities

• Assess patients and families to identify care needs.
• Develop and implement individualized care plans.
• Monitor vital signs and document progress regularly.
• Administer physician-ordered medications and therapies.
• Educate families on care techniques and symptom management.

Qualifications

On An Average Day You Will:

• Make a true difference—one patient, one family, one compassionate moment at a time.
• Develop and implement individualized care plans based on current and prior medical history
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure
• Administer physician-ordered medications and treatments with skill and empathy
• Educate patients and their families on care techniques and symptom management
• Track and communicate changes in patient condition, ensuring timely and effective responses
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery
• And contribute to other supportive duties as part of a compassionate, mission-driven team
• And additional duties as assigned

What You'll Bring to The Team:

• A heart for hospice care and a commitment to exceptional service
• A valid Registered Nurse license in good standing in the state you will work in
• A valid driver’s license and reliable transportation
• CPR certification (current)
• Strong observational and communication skills—both written and verbal
