Job summary

Bristol Hospice seeks a dedicated RN Case Manager in San Mateo County, CA to deliver compassionate, whole-person hospice care. You will advocate for patients and families, collaborate with interdisciplinary teams, and ensure comfort and dignity at every step.

The role requires an active RN license in California, CPR certification, and a valid driver's license, with daily rounds across the San Mateo area. Compensation reflects hourly salary in a supportive, mission-driven culture.

Qualifications

• Must hold a valid RN license in good standing in the state you will work in.
• CPR certification (current).
• Reliable transportation and a valid driver’s license.
• Strong observational and communication skills—both written and verbal.

Responsibilities

• Develop and implement individualized care plans based on current and prior medical history.
• Deliver attentive, one-on-one support to patients and families, ensuring comfort, dignity, and understanding.
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey.
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure.
• Administer physician-ordered medications and treatments with skill and empathy.
• Educate patients and their families on care techniques and symptom management.
• Track and communicate changes in patient condition, ensuring timely and effective responses.
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.

Schedule: Monday - Friday 8:00am-5:00pm
