Job summary

The Marion VA Medical Center in Paducah, KY seeks a full-time registered nurse to join the Primary Care Team as an RN Case Manager. You will deliver patient-centered care, assess Veterans’ needs, develop care plans, and coordinate services across the care continuum to improve outcomes.

Responsibilities include triaging data, collaborating with PACT, HBPC, Mental Health, Social Work and Pharmacy, delivering health education, and promoting self-management.

The Marion Il VMAC is seeking a?full-time?registered nurse to join the Paducah CBOC Primary Care Team

Key Responsibilities

Registered Nurse Case Manager provides patient-centered care management principles to support Veterans in managing chronic, high-risk, or post-acute conditions. The RN performs comprehensive assessments, develops individualized care plans, implements clinical interventions, and evaluates patient outcomes across the continuum of care.

The RN triages patient-generated data-including vital signs, symptom reports, and structured clinical questions-identifying potential exacerbations and initiating timely interventions. The RN collaborates extensively with interdisciplinary partners, including PACT, HBPC, Mental Health, Social Work, Pharmacy, and Nutrition, and facilitates communication with community providers when care is shared.

The RN delivers health education, coaching, and motivational counseling to enhance Veterans' self-management skills, reduce preventable hospitalizations, and improve access to care.
