Embrace Hospice is seeking a dedicated RN Case Manager to join our San Antonio team. You will oversee comprehensive patient care plans, coordinate services, and work closely with families to honor patient goals.

Requirements include RN licensure, 1 year nursing experience (3+ preferred), hospice setting experience preferred, and a commitment to documentation excellence. A valid driver's license and reliable vehicle are required.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured and in good working order.

Responsibilities

• Assess/ monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
