Qualifications

• Active Texas RN license required to practice in the field.
• Strong assessment and critical thinking skills are essential.
• Excellent communication and teamwork abilities are necessary.
• You must be able to work independently in patients' homes and facilities.
• Reliable transportation and willingness to travel within the service area.
• Demonstrated compassion, patience, and empathy when caring for patients and families.

Responsibilities

• Manage a caseload of hospice patients in the San Antonio area.
• Provide skilled nursing care in patients’ homes and facilities.
• Complete routine visits, admissions, and recertifications.
• Develop and update individualized plans of care.
• Collaborate with physicians and the interdisciplinary hospice team.
