Job summary

Adoration Hospice, a unit of Adoration Health, is seeking a dedicated Hospice RN Case Manager in Murfreesboro, TN. The role focuses on delivering compassionate, patient-centered hospice care with a strong emphasis on family education and collaboration with the care team.

You will assess patients, develop individualized care plans, administer medications, manage pain and symptoms, and ensure thorough documentation while supporting families and coordinating with a multidisciplinary team.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Must be a licensed driver with an automobile that is insured and in good working order.
• Possess and maintain CPR certification as required by state.

Responsibilities

• Assess/monitor physical, emotional, and psychosocial needs of patients.
• Create hospice care plans aligned with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management for patients.
• Educate and support patient’s family and caregivers.
• Collaborate with interdisciplinary team to coordinate care.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

Qualifications
