Adoration Health in Cleveland, MS is seeking an experienced Hospice RN to join our compassionate team. You will provide expert, patient-centered care at a critical time, coordinating with family and the interdisciplinary team to honor patient wishes.

As an RN Case Manager you will assess needs, create care plans, administer medications, manage pain and symptoms, educate families, and maintain thorough documentation.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
