Job summary

Adoration Health is seeking a dedicated Hospice RN to join our team in Cleveland, MS. You will provide expert, patient-centered hospice care in the home setting, coordinating with an interdisciplinary team and supporting families during a challenging time.

Requirements include RN licensure, 1 year nursing experience (3+ preferred), and hospice experience is a plus. Competitive benefits, mileage reimbursement, and growth opportunities are offered.

Qualifications

• Graduate of an accredited nursing program with current RN licensure in state of operation
• 1 year nursing experience required, 3+ years preferred
• Experience in hospice or similar setting preferred
• Knowledge of hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with insured vehicle in good condition

Responsibilities

• Assess, monitor physical, emotional, and psychological needs of patients
• Create hospice care plans aligned with patient wishes and goals
• Direct nursing care: administer medications, treatments, interventions
• Provide pain and symptom management
• Educate and support patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch

As a RN Case Manager You will:
