Job summary

Adoration Hospice in Murfreesboro, TN is hiring a Registered Nurse Case Manager to provide patient-centered hospice care in Williamson and Rutherford counties.

The role supports patients and families with care planning, symptom management, and coordination of interdisciplinary care from a full-time, Monday–Friday schedule based at the Murfreesboro office. You will work autonomously with team support and participate in on-call rotation as required.

Qualifications

• Graduate of an accredited nursing program with RN licensure in TN.
• One year nursing experience required; 3+ years preferred.
• Experience in hospice or palliative care preferred.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.
