Job summary

Haven Hospice in DeLand, FL is seeking a dedicated RN Case Manager to provide patient-centered hospice care in the home or assigned setting. You will assess needs, develop care plans aligned with patient goals, and coordinate nursing services with an interdisciplinary team.

Requirements include graduation from an accredited nursing program with state licensure, at least 1 year of nursing experience (3+ years preferred), and a valid driver's license.

Qualifications

• Graduate of an accredited nursing program with current licensure to practice in the state of operation.
• One year nursing experience required; 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with a vehicle that is insured and in good working order.

Responsibilities

• Assess/ monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
