Haven Hospice in Jacksonville, FL is seeking an RN Case Manager to provide compassionate, patient-centered hospice care in a home setting. You will assess needs, develop care plans, and support families while coordinating with an interdisciplinary team.

Qualifications

• Graduate of an accredited nursing program with active RN license.
• Minimum 1 year nursing experience; 3+ years preferred.
• Hospice or palliative care experience preferred.
• Valid drivers license and reliable vehicle.

Responsibilities

• Assess and monitor physical, emotional, and psychosocial needs of patients.
• Create and implement individualized hospice care plans.
• Provide direct nursing care including medications and treatments.
• Manage pain and symptoms; educate families.
• Collaborate with interdisciplinary team and document timely.

As a RN Case Manager You will:

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient's wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
